CONSULTING AGREEMENT

This is an agreement between M.A.T. Consulting, Inc, CONSULTANT, and, ______________________CLIENT.

This agreement is for professional services to be rendered by CONSULTANT for CLIENT on __________. The CONSULTANT agrees to provide services consisting of ___________________

CLIENT agrees to compensate CONSULTANT as follows:

PROFESSIONAL FEE: $000.00

EXPENSES: CLIENT agrees to pay for handouts, overheads, and related out-of-pocket expenses upon receipt of invoice.

DEPOSIT: CLIENT agrees to pay deposit of $000.00 representing one half of CONSULTANT'S fee upon execution of this agreement.

BALANCE: CLIENT agrees to pay balance of $000.00 on the date of service for each session ($000.00 per session).

In the event the CLIENT makes any changes in said facilitation date, the deposit sum of $000.00 will be retained by CONSULTANT and applied to any future presentation for a period of one year from the date CONSULTANT was notified of the change. In addition, if the change is made 30 days or less before the appearance date, CLIENT shall be responsible for reimbursing all costs incurred by CONSULTANT relating to presentation.
